All those girls who test positive are killed, girls who test negative are kept for further drug testing. This pattern leads her to Amma’s beggar mafia and the hidden operations surrounding it.

Ramanujan, played by Prajesh Kashyap, is disguised as an NGO founder, who runs a trust. In reality, it is a supply chain for something more dangerous. Shivani uncovers that NGO founder Ramanujan is running illegal drug trials using the kidnapped girls, exploiting them for experimental treatments linked to cervical cancer.
The girls abducted by Amma’s network are deliberately tested for medical trials. Those who fit are subjected to dangerous drug testing, while others are killed. The positive-negative screening detail becomes the chilling clue that exposes the deadly collaboration between Amma and Ramanujan.
In the final scene of ‘Mardaani 3’, Shivani tracks down Amma, the main antagonist behind the child trafficking network, and confronts her in Sri Lanka with the help of Fatima (played by Janki Bodiwala). During that confrontation, Amma is killed while trying to escape.
Ramanujan, who had been posing as a social worker but was actually deeply involved in the crime network, is also found and captured. After being apprehended, Shivani ensures he faces consequences for his role in the kidnappings and illegal medical experiments. At the very end, Ramanujan is left in a paralysed state in hospital after Shivani uses the cervical cancer drug on him.
