Many women experience breast soreness before their period, a condition known as cyclic mastalgia. This discomfort can range from mild tenderness to severe pain, affecting daily activities. We spoke to Dr Mannan Gupta, Chairperson & HOD, Obstetrics & Gynaecology, Elantis Healthcare, Lajpat Nagar, New Delhi, to understand its causes and effective management that can help alleviate symptoms and improve quality of life.
Causes Of Sore Breasts Before Period
According to Dr Mannan, the primary cause of premenstrual breast soreness is hormonal fluctuations during the menstrual cycle. Estrogen and progesterone, the key female hormones, fluctuate throughout the cycle, impacting breast tissue.
Estrogen Surge: In the first half of the menstrual cycle, estrogen levels rise, leading to enlargement of the breast ducts.
Progesterone Peak: In the second half (luteal phase), progesterone levels increase, stimulating the growth of milk glands, leading to swelling and tenderness.
These hormonal changes cause fluid retention in the breasts, making them feel heavier and more sensitive. Other factors contributing to breast soreness include:
- Caffeine Consumption: Stimulants like caffeine can increase breast pain.
- Dietary Factors: High salt intake can lead to fluid retention and worsening swelling.
- Hormonal Medications: Birth control pills or hormone replacement therapy can intensify symptoms.
- Stress: Increased cortisol levels may exacerbate premenstrual symptoms.
Treatment And Management
Dr Mannan highlighted that managing premenstrual breast soreness involves lifestyle changes, dietary modifications, and medical interventions.
Lifestyle Changes
- Wear A Supportive Bra: A well-fitted bra can reduce discomfort by providing adequate support.
- Reduce Caffeine and Salt Intake: Cutting down on coffee, tea, and salty foods may help decrease fluid retention.
- Regular Exercise: Engaging in low-impact exercises like walking or yoga improves blood circulation and reduces hormonal fluctuations.
Medical Treatments
- Over-the-Counter Pain Relievers: Nonsteroidal anti-inflammatory drugs (NSAIDs) can help alleviate pain.
- Vitamin Supplements: Some studies suggest that magnesium and vitamin B6 may help reduce breast tenderness.
- Topical Treatments: Applying warm compresses or over-the-counter creams with anti-inflammatory properties may offer relief.

When To See A Doctor?
Dr Mannan said, while breast soreness before a period is common and usually harmless, medical consultation is necessary if:
- The pain is severe and affects daily activities.
- There are lumps or changes in breast texture.
- The pain persists beyond menstruation.
Maintaining a balanced diet, managing stress, and staying physically active can significantly reduce discomfort.
